If you are already a MassHealth member you can Switch to HNE Be Healthy any time.

To switch to HNE Be Healthy, call the MassHealth Customer Service Center at 800.841.2900 (TTY 800.497.4648) Monday - Friday 8:00 a.m. to 5:00 p.m.

If you have not yet applied for MassHealth follow these steps:

STEP 1.

Download and fill out a Medical Benefit Request (MBR)

  • Be sure to answer all questions and fill out any sections and supplements that apply to you and your family. You may fill out the form online and print it out or print it out and fill-in the form by hand.
  • Gather your documents. You will need the following paperwork to submit with your MBR.*
    • Proof of Income
    • Immigration Status (if this applies to you)
    • Proof of Citizenship for each person applying
    • HIV Status
    • Insurance coverage ( if this applies to you)
    • Proof that you have applied for a SSN for every family member who is applying for MassHealth

* For specific documentation requirements please follow the instructions on the MBR.

  • Make a copy of everything you submit to MassHealth so you know exactly what you sent
  • File all papers in a safe place that you will remember
  • Sign and date the application after you fill it out. Send the application and all other needed papers to:

STEP 2.

Once you qualify, Choose HNE BE Healthy

Once you qualify for MassHealth, you must call the MassHealth Customer Service Center at 800.841.2900 to choose HNE Be Healthy as your health plan.

You may also go to your local health center and ask to speak to a financial counselor who can help you apply for MassHealth.
